O008 The impact of home-based exercise prehabilitation on skeletal muscle mitochondrial function in patients awaiting curative surgery for cancer

Abstract Introduction Cancer cachexia is associated with poor surgical outcomes, including increased length of stay, morbidity and mortality. Cachexia a complex multi-faceted pathological process consistently energy imbalance via hypermetabolism. Beyond this, pre-clinical animal models suggest mitochondrial dysfunction to be an important component cachexia, although this unconfirmed in humans. This study aimed investigate the function pre-surgical cancer patients, impact exercise prehabilitation on this. Methods was approved by NHS research ethics committee (IRAS: 275264). Male patients awaiting surgery curative intent for colorectal prostate were eligible, healthy volunteers recruited comparison. Muscle biopsies vastus lateralis taken before after 4-weeks home-based (3x/wk). Mitochondrial oxidative phosphorylation (OXPHOS) analysed using high-resolution respirometry (Oroboros Instruments). Results Eleven 6 recruited. had significantly lower maximal coupled (44.49pmol/(s*mg) vs. 74.81pmol/(s*mg), p=0.004) uncoupled (55.82pmol/(s*mg) 100.00pmol/(s*mg), p<0.001) OXPHOS capacity compared controls. Exercise training (44.10pmol/(s*mg) 54.64pmol/(s*mg), p=0.045) (56.20pmol/(s*mg) 71.96pmol/(s*mg), p=0.001) cohort. Conclusion Declines appear play role cachexia. Short-term training, keeping target timeframes (i.e., <31-days from decision-to-treat), appears attenuate these declines. Physical exercise and mitochondrial function in human skeletal muscle.

Muscle adaptation to endurance training involves qualitative changes in intrinsic properties of mitochondria. After training, the ADP sensitivity of miitochondrion is decreased whereas the effect of creatine on respiration is increased. This results in an improved control of aerobic energy production. Acute exercise does not adversely affect mitochondrial function.
